ICWF 2023 Application Form

Online auditing will be available and the form will be released in late spring.

Please provide a link to an online video of your conducting which includes 3-5 excerpts, each generally not less than 2 minutes or more than 7 minutes. The excerpts should be combined into one video with timing of the beginning of each excerpt included in the video description. Please post on YouTube or Vimeo and ensure that the privacy is set to Public or Unlisted. Returning participants are not required to provide a video but are encouraged to provide one that is more recent than the last workshop in which you participated.

In no more than 500 words, please explain how you hope that participation in this workshop will help you in your musical development and career.

I attest that by two weeks before the first session in which I participate, I will be fully vaccinated according to the CDC definition (two weeks after the final dose) and I can provide proof of vaccination (a copy of the completed vaccination card). I understand that it is an ICWF requirement that all in person participants be fully vaccinated in order to participate.

By checking the "Yes" box, I agree to the following:

• All of the information above is true and accurate to the best of my knowledge.
• All terms and schedule of tuition payment and that failure to fulfill the terms may result in loss of placement in the workshop and additional late fees as stated on the workshop page.
• In the case of participant cancellation no less than 90 days prior to the first day of the workshop, ICWF will return all tuition minus the tuition deposit. In the case of participant cancellation between 90 and 30 days prior to the workshop, no fees will be refunded for cancellation unless ICWF is able to find a suitable substitute, which ICWF will make good faith effort to find. There are no refunds for participant cancellation within 30 days of the start of the workshop.
• There may be changes to the schedule, repertoire, or faculty, and that AEPMS, ICWF, and/or Benjamin Loeb will not be liable for damages or be obligated to issue a refund.
• Neither AEPMS, ICWF, nor Benjamin Loeb shall be under any liability for failure to present the workshop in the event that such failure is due to acts or regulations of public authorities, labor difficulties, civil tumult, strike, epidemic, or interruption or delay of transportation service or any similar or dissimilar cause beyond the control of the ICWF. 
• I understand that AEPMS and ICWF retain all rights to all video, audio, and photographic images recorded during the workshop and may use any images without permission. 
• I understand that AEPMS and ICWF may broadcast either live or recorded any session or portion of a session online and may monetize that broadcast, and I agree that I have no rights to independently stream live any sessions or portions of sessions from the workshop.
• I understand that ICWF is an agent for AEPMS.
• I understand that ICWF will provide housing and meals as stated on the website.
• I understand that I am responsible for my own travel and all permissions to travel to the workshop and that ICWF is not responsible for reimbursing any related travel expenses in the case of cancellation or delay.
• I agree to adhere to all local, state, and federal laws during the workshop.
• I agree to adhere to all rules and policies for the hosting facility and assume full liability for any consequences for not adhering to those rules understanding that may include dismissal from the workshop with no refund of any fees.
• I agree to attend at all workshop sessions.

As part of our agreement with Avaloch Farm Music Institute, all applicants for ICWF 2023 Avaloch must agree to the following terms :

• In consideration of being chosen to participate in a program offered at Avaloch Farm Music Institute, Inc. at their facility at 16 Hardy Lane, Boscawen, NH and to use the facilities and services associated with the facility, I do hereby unconditionally and forever waive, release and discharge, and agree not to sue, Avaloch Farm Music Institute, Inc., and its agents, employees, representatives, executors and all others acting on its behalf from any and all claims or liabilities for injuries or damages to my person and or property (ies), including musical instruments of any kind.

• I declare myself to be physically healthy and sound and suffering from no condition, impairment, disease, infirmity or other illness that would affect my participation in the program. I understand that all of the facilities are ADA compliant.  I declare that I have no allergies to food or other materials that would affect my stay or participation at the facility unless noted in separate Health and Allergies form.

• I am aware that there is no obligation for any person to provide me with medical care during my stay at the facility and that there are no trained medical personnel available at the facility. If medical care is necessary and is rendered to me, I consent to that care in advance if I am unable to give my consent for any reason at the time the care is rendered.

• I understand that the facility encompasses a lake with access to paddle/rowing and canoe boats. I am aware that use in any manner of the lake or boats is at my own risk, and that no provision has been made for monitoring of the lake and boats activities by Avaloch Farm Music Institute or their representatives.  I fully assume responsibility for any risk of accident or physical injury, death, property damage and all harm or damage that may be caused to myself or others due to my use of the lake facility, including swimming, or canoes or paddle/rowing boats and agree to hold harmless Avaloch Farm Music Institute Inc and all others acting on their behalf.  

• I fully assume responsibility for any risk of accident or physical injury, death, property damage and all harm or damage that may be caused to myself or others due to my participation in any official or unofficial activities, events or trips.

• I hereby confirm that, if I have use of an automobile on the premises, I have confirmed that there is proper insurance coverage in place and agree to hold harmless and indemnify Avaloch Farm Music Institute, Inc. and its representatives for any and all loss arising out of the use of said automobile

• I hereby grant permission to Avaloch Farm Music Institute, Inc. and its sponsors, transferees, associates, representatives, agents and licensees to utilize any photographs, motion pictures, videotapes, recordings and any other references or records of my participation at the facility which may depict, record or refer to me for any purpose, including commercial use, by Avaloch Farm Music Institute, its sponsors, transferees, associates, representatives, agents or licensees.

• I hereby agree that if any portion of this Agreement is held invalid or unenforceable, the remainder of the Agreement shall remain in full legal force and effect.

• I hereby understand that the minimum drinking age in New Hampshire is 21 years of age. I agree that if I am not yet 21 years of age I will refrain from drinking while attending the Avaloch Farm Music Institute. I agree to not make false statements about my age or to make alcohol available to someone underage.  I further agree to comply with the laws of the State of New Hampshire and if I do not I understand that I may be expelled from the Avaloch Farm Music Institute. 

After you submit your application, you will be directed to a page where you pay the application fee. Your application will not be completed and processed unless the fee is paid.
